How to Register for the Fall 1999 Semester (October 25, 1999 - January 9, 2000)
The registration procedure for this course involves three steps:
1. Submit a completed Registration Form.
2. Pay the course fee.
3. Send a confirming e-mail.
You will be sent the course workbook in the mail within two weeks of receipt of the fee and Registration Form, provided they are received at least two weeks prior to the class commencement date.
Payment of Course Fee
The basic course fee is $475.00. Remember to add shipping if you are outside the U.S. or Canada, and sales tax if you're a California resident). The fee is payable in full upon registration. There are three methods of payment:
1. US Check
Check drawn on US bank, made payable to Jeremy Sutton. Mail to the address at the bottom of the Registration Form.
2. International Money Order
Make the Money Order payable to Jeremy Sutton. Mail to the address at the bottom of the Registration Form.
3. Visa or Mastercard
Fill in the card details below and send in the signed completed Registration Form.
You can print this form out and then either fax it (415-626-3901) or mail it to me (address below). In either case please also send a confirming e-mail stating when and how you sent payment.
